Diversus Health offers TST Outpatient Services and Intensive Outpatient Programming (IOP).

Outpatient services are provided for youth ages 10 - 18 onsite or via telehealth, Monday through Friday and include individual & family therapy, safety planning, family education and support, skills training and development, case management, and psychiatry services.

Intensive Outpatient Program (IOP) provided for youth ages 12-18, Monday - Thursday onsite from 8:30am to 11:30am for 2 weeks.

Does the patient meet the following criteria to determine if case is appropriate for Trauma Systems Therapy services referral?

Inclusion Criteria

• Ages 10 - 18 years
• History of complex trauma
• Trauma is suspected to be the primary reason for current symptoms and impaired functioning
• Impaired functioning across multiple settings (i.e., home, school, peer relationships, work)
• History of engagement in traditional outpatient therapy services with limited benefit
• Family/Caregiver are willing to participate in therapeutic services that include them
• Evidence of specific, definable survival-in-the-moment states. Survival-in-the-moment states are defined as: an individual's experience of the present environment as threatening to his or her survival which results in ineffective behavioral or emotional responses.

Exclusion Criteria

• Diagnosis of Significant Psychosis
• Intellectual developmental delays
• Autism Spectrum Disorder - may require additional screening to ensure TST is the appropriate modality
• Early Childhood Treatment needs - we do not provide this within TST so younger children need to be mature and able to cognitively engage in the assessment and treatment delivery process.

Each of our patients starts with an evaluation to determine the best stage of treatment in our recovery program suited for each individual on a case-by-case basis.

Our addiction counselors are here for you. In addition to our substance use disorder (SUD) services, we provide DUI education and therapy for those who are mandated and within the criminal justice system. We offer urinalyses, drug monitoring, and testing compliant services for chain of custody cases. All our SUD services are regulated by the Colorado Department of Human Services, Office of Behavioral Health (OBH).

There are no disqualifying factors for the SUD program. If you or someone you love are seeking treatment for a substance use concern, this program is for you.

Ascent is designed to be an early intervention program that provides specialized, coordinated services for those with recent onset of Schizophrenia symptoms or clients who are at risk for the development of Schizophrenia. The program lasts for three years and is designed to increase independence while helping clients return to their preferred life path before the onset of psychosis. Due to this, the following criteria must be met before considering a patient for Ascent:

• The patient is between the ages of 15 and 29.

• The patient has a primary diagnosis of Schizophrenia, Schizophreniform Disorder, Schizoaffective Disorder, Brief Psychotic Disorder, Unspecified Schizophrenia or Other Specified Schizophrenia

• The onset of psychosis or the patient meeting full criteria for Schizophrenia is less than two years.

• The patient's psychosis is not a feature of Bipolar Disorder or Major Depressive Disorder, it does not have a primary cause related to trauma, the psychosis exists outside the use of substances and it is not medically induced

• The patient does not have a developmental disorder that leads to significant impairment in functioning (defined as leading to an IQ of less than 70)

• The patient is considered safe for our staff to meet in the community and is not currently presenting with aggressive or threatening behaviors.

ACT is designed to be a program that provides a higher level of care in the community for patients with a psychotic disorder that do not respond to traditional outpatient services. ACT exists to provide stability to patients with a high level of utilization of inpatient services. Due to this, the following criteria must be met prior to considering a patient for ACT:

• The patient has a psychotic disorder as their primary diagnosis.

• The patient's psychosis is not due to Major Depressive Disorder with Psychotic Features, a trauma-related disorder, a medical condition or a developmental disability.

• The patient is 18 or older

• The patient has agreed to ACT services, including meeting with a psychiatric provider. The patient is aware that ACT services are intensive and typically involve at least three appointments with the team per week

• The patient has experienced at least two hospitalizations within a twelve-month period

• The patient is considered safe for our staff to meet in the community and is not currently presenting with aggressive or threatening behaviors
